To complete the requirements for an Students First Foundation Scholarship for 2019/20, the parent or guardian must provide the following information:
Signed and Completed Application
Household Income Information section
Signed and Completed School Participation section
Public School Attendance Verification Form (If Applicable)
Proof of Residency Documents (i.e. most current State Tax Return, lease, electric bill)
Documentation of Income (i.e. last 30 days pay stubs, 1099, W-2, Tax Returns)
A copy of your tax returns is not required.
