Scholarship Verification Form

To complete the requirements for an Students First Foundation Scholarship for 2020/21 school year, the parent or guardian must provide the following information:

  • Signed and Completed Application
    • Household Income Information section
    • Signed and Completed School Participation section
    • Public School Attendance Verification Form (If Applicable)
  • Proof of Arizona Residency Documents (i.e. current State Tax Return, lease, electric bill)
  • Documentation of Income (i.e. last 30 days pay stubs, 1099, W-2, Tax Returns)
               A copy of your tax returns is not required.
